**Use with a clinician-prescribed Envision™ Pro continuous glucose monitoring (CGM) device**

The Envision™ Pro CGM system is a disposable, zero calibration continuous glucose monitoring (CGM) system that continuously records glucose data, sends it to the healthcare provider, and produces comprehensive reports to create personalized diabetes care plans. The Envision™ Pro app (MMT-7782) is a component of the Envision™ Pro CGM system. The app collects the information recorded during an Envision™ Pro CGM evaluation, and sends the data to the healthcare professional. The app has both a patient and healthcare professional mode.

PATIENT MODE:

• Simply record daily activities such as meals, exercise, medications and sleep while undergoing the Envision™ Pro CGM evaluation

• The app collects and uploads data from the Envision™ recorder

• At the end of your evaluation, the uploaded data can be used in reports to review with your healthcare professional

HEALTHCARE PROFESSIONAL MODE:

• Use when your patients do not have a smartphone

• Allows you to initiate and end evaluations

• See a list of your patients’ undergoing evaluations

• Collect and upload data from the Envision™ recorder
