Where cause is Surgical Complication now requests surgical history input.
File names for Send Data and Export Clusters include area name, area code. Export Clusters file name also includes cluster number.
Setup New Survey is one-time setup only to prevent accidental switching between RAAB5 / RAAB6 configurations.
