Majority with hypercholesterolemia in the absence of positive family history or secondary cause(s) have polygenic hypercholesterolemia. By contrast, familial hypercholesterolemia (FH) is monogenic disorders affecting the clearance rate of LDL-C from the blood. To date, mutations in four genes (LDLR, APOB, PCSK9, and LDLRAP1) have been identified causing the disorder.

Persons with documented CVD, type 1 or type 2 diabetes, very high levels of individual risk factors (e.g. familial hypercholesterolemia) are automatically at very high or high total CV risk. No risk estimation models are needed for them; they all need active management of all risk factors.

For all other people, the use of a risk estimation system such as SCORE is recommended to estimate total CV risk since many people have several risk factors that, in combination, may result in unexpectedly high levels of total CV risk.
