The NBA 2K18 is a basketball simulation which, like the previous apps in this series, seeks to realistically portray the National Basketball Association (NBA), as well as the recent fixes from previous installments. Players primarily play NBA apps with real players or teams; apps follow the rules and goals of finding NBA apps. Some game modes are present and many settings can be customized. In terms of comments, Kobe Bryant and Kevin Garnett appeared as guest commentators.

A principal of the series, MyCareer, returns as one of the available game modes. MyCareer is a career mode where players create customizable basketball players and play through their basketball career. This mode has a storyline that is played when players compete in the app and also off the field. MyGM mode tries to introduce more cutscene style interactions than previous apps in an effort to give storyline mode, dubbed 'Next Chapter', similar to series' MyCareer mode. In addition, some additions have been made to the fashion that meets the latest NBA Collective Bargaining Agreement, and the NBA G League, the official NBA league basketball organization, is present.
