Community Health Connections is here to offer easy access to quality health care to whoever walks through our doors. We want that same access to be easily available in virtual form, too. My CHC is your direct connection to your medical, behavioral health, and optometry care, anytime, anywhere*.

With My CHC you can:

PREPARE FOR YOUR APPOINTMENTS

Fill out forms, sign documents and pay your copay from home. This will speed up your check-in process at the office and save you time.

COMMUNICATE WITH YOUR CARE TEAM

Whether you have a question for your care team, you can easily send a secure message to the office using the app.

STAY UP-TO-DATE ON YOUR CARE

View and pay your medical bills, set reminders for your health “to-dos”, read news from Community Health Connections, view updates to your health record, and receive notifications for upcoming care needs.

To access My CHC, download the app and create an account with a username and password.
